Getting Started with an Appointment

2. Contact your insurance company. Medicare patients will need an order from their primary care physician for many of the specialized testing (hearing evaluation, VNG or Tinnitus). Many other
   private insurance companies may also require an order.

3. Print and fill out required paperwork. Required paperwork includes

Specialized testing may require additional paperwork. Please ask the front office person for required paperwork.

4. Please arrive 10-15 minutes before your scheduled appointment.
